An overview of the credentialing requirements needed on TPN.match

Your Guide to Credentialing Requirements

To ensure our behavioral health network meets the highest professional, ethical, and regulatory standards, TPN.health maintains a thorough credentialing process. This guide outlines exactly what you need to provide when completing your TPN.match credentialing profile, so you can move through our verification queue as smoothly as possible.

1. Primary Source Verification (PSV)

We conduct Primary Source Verification (PSV) for every clinician. This means TPN.health independently verifies your license status and standing directly with your state licensing board rather than relying solely on the information you self-report.

  • What to provide: You must input your license number, license type, issuing state, and expiration date in the portal.

  • Requirements: You must hold a current, unrestricted professional license in each state where you render services.

2. Controlled Substance Registration (DEA)

This is only required for prescribing providers (e.g., psychiatrists, psychiatric nurse practitioners).

  • What to provide: If you prescribe controlled substances, select "I prescribe controlled substances" in the portal. You will need to enter your active DEA registration number and registration expiration date.

  • Ongoing compliance: DEA registrations are re-verified every 90 days.

3. Professional Liability (Malpractice) Insurance

All contracted providers are required to maintain active malpractice coverage.

  • What to provide: Upload a current Certificate of Insurance (COI) in PDF format and input your policy’s expiration date.

  • Minimum Coverage Thresholds: Professional Liability: $1,000,000 per occurrence / $3,000,000 aggregate. Commercial General Liability: $1,000,000 per occurrence / $3,000,000 aggregate.

4. Comprehensive 5-Year Work History

We require a complete work history evaluating the continuity of your professional experience.

  • What to provide: Detail your professional activities over the past five (5) years—including employment, private practice and/or volunteer work.

  • Explaining Gaps: You must identify and provide a written explanation for any gap in employment of 30 days or longer during this five-year window. (Please note: our internal agents are instructed to flag unexplained gaps, so providing clear reasons during setup prevents unnecessary delays).

5. Background Disclosure & Attestation

You must complete a standardized Background Disclosure Questionnaire electronically within the portal.

  • What it covers: Questions address your disciplinary history, malpractice claim history, federal program exclusions, and practitioner attestation regarding your ability to practice safely.

  • Please note: Honesty is critical. Material misrepresentation or omission on this questionnaire is grounds for immediate termination from the network, regardless of the underlying event.

  • Ongoing compliance: This questionnaire is re-administered every two (2) years.

What Happens Next?

Once you submit your profile, our Credential Verification Agents will review your file section-by-section.

• No issues found: Your profile will be approved and routed directly to our Credentialing Committee for final sign-off.

• Incomplete info or gaps: If there are missing details, unlogged DEA matches, or unexplained employment gaps, your file may be put in "Awaiting Provider" status or flagged for clinical review, which will pause your onboarding. We will contact you if we need additional information.
